New Client Paperwork:
If you are a new client, you will receive a welcome email once you schedule your first appointment. This email will provide instructions on setting up a client portal through Therapy Notes. All paperwork will be sent to you through the portal. All paperwork needs to be completed at least 48-hours prior to the appointment. We can make accommodations for anyone who does not have internet access or prefers forms to be emailed or made available in office prior to the appointment.

Coordination of Care:
If you would like your to coordinate care with another provider (for example, your psychiatrist, school, etc.), please complete the release of information provided to you upon intake in your therapy portal. If you need an updated or new release, please notify your therapist. This can be completed in session or sent to you through the portal at any time.
